You should consult either the Chicago Manual of Style or Turabian's Manual for Writers for full guidlines on how to format your footnotes and bibliography if you are writing a dissertation in Chicago or Turabian style.

MLA Style Dissertation Formatting

MLA Style, or Modern Language Association Style, is commonly used in the humanities, particularly in fields that deal with literature and language. The MLA Handbook for Writers of Research Papers is the go-to reference source for users of MLA Style. Like APA Style, MLA Style uses in-text citations. In MLA Style, the reference list is referred to as the Works Cited page. You should consult the MLA Handbook for guidelines on how to properly format citations and the Works Cited page.

So . . . Which Style Guide Should You Use For Dissertation Formatting?
If your department or institution doesn't specify which style to use, find out what is standard in your field.
